Are you evaluating a patient for leptospirosis?

Does the patient have a headache of sudden onset?

Has the patient had fevers?

Maximal body temperature

Does the patient have bilateral conjunctival suffusion?

Does the patient have meningism?

Does the patient have muscle pain, typically in the calf muscles?

Is the patient jaundiced?

Does the patient have albuminuria?

Does the patient have an elevated BUN?

Has the patient had contact with animals at home, work, leisure or on travels?

Has the patient had contact with known or possible contaminated water?

Is a culture or PCR test positive for leptospira?

Is the patient from an area where leptospirosis is endemic?
