Parameters:
(1) greatest dimension in centimeters
(2) number of risk factors
(3) invasion into orbit, maxilla, mandible and/temporal bones
(4) invasion into other bones
(5) direct perineural invasion of skull base
Risk factors:
(1) thickness > 2 mm
(2) Clark level 4 or 5 (into mid-deep reticular dermis or into subcutaneous fat)
(3) perineural invasion
(4) primary site in ear or non-hair bearing lip
(5) poorly differentiated
