Description

A trigger tool can be used to identify patients who may have an adverse drug event (ADE). It can also be used to prospectively to identify patients who may be at increased risk for developing an ADE. This can function as an early warning system.


 

Laboratory triggers may include:

(1) INR > 5 (or 6)

(2) aPTT > 100 seconds

(3) high or rising serum creatinine (> 2 times above baseline)

(4) serum glucose < 50 or > 150 mg/dL

(5) hyperkalemia or hypokalemia

(6) hyponatremia

(7) peak serum drug level toxic

(8) trough serum drug level above target or subtherapeutic

(9) low serum albumin (< 3 g/dL)

(10) leucopenia (WBC count < 3,000 per µL)

(11) thrombocytopenia (platelet count < 50,000 per µL)

(12) abrupt drop in hemoglobin > 25%

(13) positive D-dimer

(14) positive Clostridium difficile toxin test

(15) elevated liver function tests (AST and/or ALT)
