Gattinoni et al proposed the alactic base excess (ABE) when evaluating a patient with sepsis.
Parameters:
(1) base excess in mmol/L
(2) blood lactate concentration in mmol/L
alectic base excess in mmol/L =
= (base excess) + (blood lactate)
Alactic Base Excess
Designation
< - 3 mmol/L
negative
-3 to 3.9 mmol/L
neutral
>= 4 mmol/L
positive
Interpretation:
• A plot of the alactic base excess to mortality shows a "U"-shape, with the minimum from 0 to 4 mmol/L.
• A negative alactic base excess is associated with an increasing trend in-hospital mortality as the ABE becomes more negative.
• A high alactic based excess is also associated with an increasing trend in-hospital mortality as the ABE increases.
