Description

Altamirano et al reported a number of predictors for acute kidney injury (AKI) and early mortality in a patient with alcoholic hepatitis. These can help to identify a patient who may benefit from more aggressive management. The authors are from the University of Barcelona, Centre Esther Koplowitz and Centro de Investigacion Biomedica en Red de Enfermedades Hepaticas y Digestivas in Barcelona, Spain.


Patient selection: alcoholic hepatitis

 

Parameters:

(1) acute kidney injury (AKI)

(2) ABIC score (age, bilirubin, INR, creatinine)

(3) hepatic encephalopathy

(4) systemic inflammatory response (SIRS)

 

Risk factors on admission associated with development of AKI

(1) presence of SIRS

(2) total serum bilirubin

(3) INR on admission

 

Predictors of 90-day mortality on multivariate analysis:

(1) AKI

(2) ABIC score

 

Predictors of 90-day mortality on univariate analysis:

(1) AKI after admission

(2) encephalopathy on admission or during hospitalization

(3) infection during hospitalization

(4) ABIC score on admission

(5) SIRS at admission
