Prognostic factors
(1) higher Trouillas grade (Grade 2 or 3)
(2) > 2 mitoses per 10 high power fields (hpf)
(3) >= 3% Ki-67 staining
(4) p53 expression
(5) tumor invasion
(6) ACTH subtype
