Description

Husain et al developed an algorithm for identifying a patient who may have nonalcoholic fatty liver disease (NAFLD). This can be implemented to run on data in the electronic health record. The authors are from Baylor College of Medicine and the Michael BeBakey Veterans Affairs Medical Center in Houston.


 

Patients: VA system (implies adult)

 

Criteria for a diagnosis of NAFLD:

(1) >= 2 elevated serum ALT values

(2) ambulatory setting

(3) elevated serum ALT values taken > 6 months and <= 2 years of each other

(4) exlusion of other causes

 

Exclusions specified in paper:

(1) metastatic cancer

(2) serologic evidence of HBV

(3) serologic evidence of HCV

(4) alcohol abuse (no positive AUDIT-C score within 1 year an elevated ALT)

 

Other causes of hepatitis also need to be excluded (not mentioned in paper):

(1) anabolic steroids

(2) medications

(3) autoimmune hepatitis

(4) primary biliary cirrhosis

(5) other causes of liver disease (hemochromatosis, etc)
