Description

Some patients with ANCA-associated vasculitis may develop bronchiectasis.


Presentation:

(1) cough

(2) dyspnea

(3) hemoptysis

 

ANCA-associated vasculitis subtypes associated with bronchiectasis include:

(1) microscopic polyangiitis (MPA)

(2) granolumatosis with polyangiitis (GPA)

(3) eosinophilic granulomatosis with polyangiitis (EGPA)

 

ANCA-specificity:

(1) myeloperoxidase (MPO, 64%)

(2) proteinase-3 (21%)

 

Factors associated with worse survival:

(1) diagnosis of bronchiectasis prior to the diagnosis of ANCA-associated vasculitis (HR 5.8)

(2) microscopic polyangiitis (HR 18.1)

(3) absence of ENT involvement (ENT involvement associated with HR 0.1)

(4) older age at diagnosis of vasculitis (HR 1.3)

 

Patients with the diagnosis of bronchiectasis preceding the diagnosis of vasculitis tended to have:

(1) mononeuritis multiplex

(2) microscopic polyangiitis

(3) MPO specificity

(4) a worse survival
