Description

Rare strains of Neisseria meningitidis serogroup Y have been identified with antibiotic resistance. It is important to perform antibiotic susceptibility on clinical isolates.

Resistance may consist of one or both of the following:

(1) beta-lactamase

(2) ciprofloxacin resistance

Presentation:

(1) prophylaxis failure

(2) treatment failure

(3) local outbreak

(4) resistance on drug-susceptibility testing

Ceftriaxone and cefotaxime may be used for empiric therapy prior to the availability of susceptibility test results.

For prophylaxis, azithromycin, a single injection of ceftriaxone or a 2-day course of rifampin may be effective.

Vaccination against Neisseria meningitidis may reduce spread of the resistant strain
