Rarely an autoimmune disease may be associated with myelofibrosis.
Features of AIMF:
(1) pancytopenia
(2) teardrop poikilocytosis and leukoerythroblastosis
(3) diffuse fibrosis in the bone marrow biopsy
(4) evidence of SLE or other autoimmune disease
(5) exclusion of other causes of myelofibrosis
(6) reversal on appropriate therapy for the autoimmune disease
Marked splenomegaly should be absent.
Differential diagnosis:
(1) primary myelofibrosis
(2) hairy cell leukemia
