Peak body temperature

Duration of neutropenia

Number of days when resolution of neutropenia is expected

Is there evidence of early bone marrow recovery?

Is the malignancy in remission?

Is the chest radiograph normal?

Does the patient have an intravenous catheter-site infection?

Does the patient have pneumonia or other deep organ infection?

Does the patient have neurological or mental changes?

Does the patient appear ill?

Does the patient have abdominal pain?

Is the patient hypotensive or in shock?

Does the patient have hypoxia?

Does the patient have vomiting and/or diarrhea?

Does the patient have any other significant comorbid complication?

White blood cell count

Percent neutrophils in the differential count (enter from 0 to 100)

Percent monocytes in the differential count (enter from 0 to 100)

Are liver function tests normal or nearly normal?

Are renal function tests normal or nearly normal?
