Hoyme et al outlined the information needed to evaluate a pediatric patient for features of the Fetal Alcohol Spectrum Disorders in 2005. The authors are from multiple institutions in the United States and South Africa.
Patient selection: child being evaluated for a fetal alcohol spectrum disorder (FASD)
Checklist for evaluating a pediatric patient for FASD:
(1) evidence of prenatal alcohol exposure
(2) key dysmorphic features (short palpebral fissures, smooth philtrum, thin vermilion border to upper lip)
(3) neuropsychological/neurobehavioral evaluation
(4) growth retardation (height, weight, occipitofrontal head circumference)
(5) birth defects associated with alcohol exposure
(6) consideration of other conditions in the differential diagnosis

key dysmorphic features, neuropsychological findings and all 3 growth measures <= 10th percentile
fetal alcohol syndrome
