Augustinus et al reported risk factors for a chyle leak after a pancreatoduodenectomy (Whipple procedure). The presence of a chyle leak can result in the need for additional therapy. The authors are from multiple institutions in the Netherlands.
Patient selection: status post pancreatoduodenectomy
Rate of chyle leak in study: 7% for grades B and C (rare)
Predictors of chyle leak from multivariate analysis:
(1) vascular resection (OR 2.1)
(2) open surgery (OR 3.5)
Other factors associated with chyle leak:
(1) resectable tumor on preoperative assessment
Consequences if Chyle leak grade B or C:
(1) prolonged hospital stay > 14 days (OR 2.8)
