The sinusoidal obstruction syndrome (SOS) may develop in a patient following cytoreductive therapy or bone marrrow transplant. It is often clinically mild but can be life-threatening.
Synonym: veno-occlusive disease of the liver
Clinical findings:
(1) right upper quadrant pain with hepatomegaly
(2) jaundice with a total serum bilirubin > 34 µmol/L (>= 2 mg/dL)
(3) sudden weight gain (> 2% of body weight)
(4) portal hypertension with ascites
(5) hepatocellular necrosis
Organ failures that may develop:
(1) heart failure
(2) acute renal failure
(3) thrombocytopenia
(4) hepatic
(5) CNS (confusion or disorientation)
(6) sepsis
In severe disease the patient may show:
(1) multi-organ failure
(2) liver failure with marked elevation in transaminases (> 750 IU/L) and hyperbilirubinemia
