Description

A yellow fever vaccination certificate must be valid if it is to be official.


 

Requirements for a valid certificate:

(1) use of a vaccine approved by the World Health Organization (WHO)

(2) administered at an approved center

(3) form signed by the health care provider who supervised the administration

(4) stamped with the official stamp of the administering center

(5) valid date range:

(5a) from (date administered) + (10 days)

(5b) to (date administered) + (9 days) + (10 years)

(6) international certificate of vaccination or prophylaxis form fully completed in English or French (with or without another language on the same document)

(7) positive identifiers of the person vaccinated
