An inactive hepatitis B viral (HBV) carrier can be defined two ways.
Patient selection: hepatitis B virus
Recommended testing:
(1) FibroTest
(2) ActiTest
Criteria for inactive carrier - both of the following:
(1) FibroTest <= 0.27
(2) ActiTest <= 0.29
A viral load < log5 indicates a silent carrier, while a viral load >= log5 indicates an immuno-tolerant patient. Both groups had 0 complications at 4 years in the study of Ngo et al.
Alternative the diagnosis can be made if >= 3 serum ALT <= 40 IU/L.
As reported by Chao et al, a low serum ALT may be associated with significant hepatic fibrosis.
