Patient selection: evidence of sepsis and organ dysfunction (with SOFA score >= 2)
Criteria for septic shock - all of the following:
(1) vasopressors required to maintain the mean arterial pressure >= 65 mm Hg
(2) serum lactate > 2 mmol/L
(3) adequate fluid resuscitation
