Meliodosis can be a serious infection with a significant risk for relapse. A severe infection requires more aggressive management.
Severe meliodosis involves one or more of the following:
(1) septic shock
(2) deep-seated organ abscesses (including prostate)
(3) extensive lung disease
(4) septic arthritis or osteomyelitis
(5) cerebral/neurologic meliodosis
where:
• A person who is immunocompromised may be at increased risk for severe meliodosis.
Management of severe meliodosis requires:
(1) prolonged parenteral antibiotic therapy (4 or more weeks) during the initial intensive phase
(2) prolonged oral therapy during the eradication phase
(3) closer monitoring and prolonged follow-up
