Morgenstern listed several criteria for identifying a nonparasitic splenic cyst that can be managed nonoperatively (conservatively). The author is from Cedars-Sinai Medical Center and UCLA School of Medicine in Los Angeles.
Patient selection: nonparasitic splenic cyst
Criteria for nonoperative management - all of the following:
(1) small size (<= 5 cm in diameter)
(2) totally asymptomatic
(3) imaging features consistent with benign splenic cyst (all of the following)
(3a) absolutely regular cyst wall (smooth without irregularities)
(3b) absence of a solid component anywhere in the cyst
(3c) no contour defects around the cyst margin
These criteria can be modified based on specific individual features, such as recurrent trauma, presence of a known malignancy, etc.
