Have cryptosporidia been demonstrated in stools or intestinal biopsy?

Has the patient noticed a change in stools frequency or form?

Duration of diarrhea

Number of daily bowel movements during diarrhea

Is diarrhea controllable with antimotility agents?

Has the patient required rehydration?

• right upper quadrant pain, jaundice, and/or elevated serum alkaline phosphatase?

• gastritis?

• pancreatitis?

• otitis or middle ear effusion?

• cough, dyspnea, and/or chest pain?
