Preexposure prophylaxis (PrEP) is an effective method of reducing the acquisition and transmission of HIV. Discontinuation of its use while at high risk is not uncommon in at-risk populations.
Use of the term discontinuation:
(1) expected discontinuation, when no longer needed due to a stable relationship or low-risk environment
(2) poor adherence to regimen, with early discontinuation
(3) stoppage of its use or initiation, which may be partial, temporary, or complete
Reasons for stoppage of use:
(1) side effects, real or perceived
(2) insurance coverage or financial problems
(3) availability
(4) access to provider
(5) psychological perceptions and beliefs
(6) substance abuse
(7) lack of awareness about need, including misperception of risk
Psychological perceptions are a complex mixture of beliefs and attitudes towards sexuality and perception of identity. Perceptions may include bugchasing, which eroticizes risk of HIV exposure.
