Varices may affect the jejunum or ileum but these only rarely cause significant bleeding. When bleeding does occur it can be difficult to locate.
The diagnosis is usually considered in a patient with gastrointestinal bleeding but negative upper and lower endoscopies.
Clinical triad:
(1) portal hypertension
(2) history of previous inta-abdominal surgery
(3) hematochezia (passing fresh blood per anum) without hematemesis
Imaging studies that can be used to monitor the patient at greater risk for bleeding:
(1) CT scan
(2) double balloon enteroscopy
(3) capsular endoscopy
Bleeding from the varices can be significant.
