Description

Wade et al developed 2 simple equations for predicting the Barthel Activies of Daily Living (ADL) Score for a patient at 6 months after an acute stroke. These can help identify a patient who may benefit from more aggressive management. The authors are from the Frenchay Hospital in Bristol, England.

Parameters:

(1) urinary incontinence at initial assessment

(2) motor deficit arm

(3) sitting balance

(4) hemianopia

(5) age in years

(6) initial Barthel ADL score

Parameter

Finding

Points

urinary incontinence

not incontinent

1

some incontinence (<= 1 time in 24 hours)

2

usually incontinent (> 1 time in 24 hours)

3

motor deficit arm

normal motor activity and function

1

some motor deficit, some function deficit

2

some motor deficit, no function

3

complete motor and function deficit (no movement at all)

4

sitting balance

normal (able to sit for 1 minute on side of bed without support despite attempt to displace)

1

slightly abnormal (can sit for 1 minute on side of bed, but falls over with an attempt to displace)

2

severely abnormal (unable to sit on side of bed for 1 minute without support)

3

hemianopia

normal

1

visual inattention

2

hemianopia

equation 1 for predicting 6-month Barthel score =

= 151 - (4.3 * (points for urinary incontinence)) - (5.9 * (points for motor deficit arm)) - (2.8 * (points for sitting balance)) - (2 * (points for hemianopia)) - (0.5 * (age in years))

equation 2 for predicting 6-month Barthel score =

= 100 + (0.31 * (initial Barthel score)) - (2.6 * (points for hemianopia)) - (0.4 * (age in years))
