A patient with congenital long QT syndrome may be able to participate in exercise under certain circumstances.
Patient selection: congenital long QT syndrome
Symptomatic: seizures, syncope, cardiac arrest
Contraindications:
(1) symptomatic LQTS1: engagement in competitive swimming or diving into cold water
(2) symptomatic and QTc prolonged (>= 470 ms for male; >= 480 ms for female)
Precautions may include:
(1) treatment with beta-blocker
(2) avoidance of QT-prolonging drugs
(3) electrolyte replenishment
(4) avoidance of dehydration
(5) avoidance of hyperthermia
(6) personal AED
(7) emergency action plan
(8) avoidance of loud or sudden noises in LQT2
The final decisions are based on shared decision making.
