Hypogonadotropic hypogonadism can rarely occur in a male endurance runner. This may be the male counterpart of the female athlete triad.
Typical patient: young male endurance runner, with intense exercise starting prior to sexual maturity
Presentations may include:
(1) infertility, erectile dysfunction or decreased libido
(2) osteopenia with fracture
(3) depressed mood
Laboratory findings:
(1) low serum LH and FSH
(2) low serum testosterone
It is important to exclude other causes of hypogonadotropic hypogonadism.
Therapeutic options may include testosterone replacement or clomiphene citrate.
