Exercise-induced vasculitis is a benign condition that can occur after exercise, especially in hot weather.
Clinical features:
(1) presence of purpuric or erythematous papular rash over feet and lower legs
(2) onset after physical activity (playing golf, prolonged walking/hiking, dancing, swimming)
(3) variable pain, pruritis, burning
(4) resolves spontaneously with rest
It may overlap with exercise-induced purpura.
Risk factors:
(1) age > 50 years
(2) hot weather
(3) prolonged exertion
Biopsy of the rash may show:
(1) leukocytoclastic vasculitis
(2) urticarial vasculitis
(3) erythrocyte extravasation
EIV may occur in a patient with an autoimmune disease and may precede the latter's recognition.
The diagnosis requires exclusion of other conditions including Henoch-Schonlein purpura and contact allergies.
