Potassium by fingerstick may be falsely elevated, sometimes to very high levels.
Mechanism: either hemolysis or leakage of potassium from erythrocytes. Other markers of hemolysis such as LDH may not be elevated.
Use: A normal or low fingerstick potassium can exclude hyperkalemia but otherwise cannot be interpreted. An elevated serum potassium cannot be interpreted.
