Rule of 10 for Initial Fluid Resuscitation in Severely Burned Adult Combat Casualty

Purpose: To determine the initial resuscitation fluid requirements for an adult with significant burns using the rule of 10 of Chung et al.

Description

Chung et al developed a simple rule for determining the fluid resuscitation requirements for a severely burned adult. The authors are from the U.S. Army Institute of Surgical Research at Fort Sam Houston and the University of Texas Health Science Center at San Antonio in Houston.


 

Resuscitation fluid: Ringer's lactate

 

Significant burn area: in percent from 0 to 100

 

Patient selection: adult >= 40 kilograms (88 pounds)

 

rounded value for burn size in percent total body surface area =

= ROUND((estimate total BSA with significant burns) / 10) * 10

 

For a patient weighing from 40 to 80 kg:

 

fluid requirement for the first 24 hours in mL/h =

= 10 * (rounded value)

 

For a patient weighing > 80 kg

 

fluid requirement for the first 24 hours in mL/h =

= 10 * (rounded value) + (((weight - 80) / 10) * 100)

 

where:

• For a patient weighing > 80 kg, the additional fluid for resuscitation could be determined by ROUND, INTEGER or the actual value as shown.
