Bettinger et al reported the Freiburg index of post-TIPS survival (FIPS) to evaluate a patient with cirrhosis undergoing a transjugular intrahepatic portosystemic shunt. This can help to identify patients likely to benefit from the procedure. The authors are from the University of Freiburg in Germany.
Patient selection: cirrhosis, possible candidate for transjugular intrahepatic portosystemic shunt (TIPS)
Parameters:
(1) serum total bilirubin in mg/dL
(2) serum creatinine in mg/dL
(3) age in years
(4) serum albumin g/L
Parameter
Poinra
serum total bilirubin
1.43 * LOG10(bilirubin)
serum creatinine
-1.71 / (creatinine)
age in years
0.02 * (age)
serum albumin
-0.02 * (albumin)
total score =
= SUM(points for all of the parameters)
Interpretation:
• A FIPS >= 0.92 is associated with high risk.
The FIPS can be used to predict overall survival for various time periods.
Period
S0
1 month
0.96
3 months
0.87
6 months
0.81
overall survival =
= (S0 for period)^(EXP((FIPS)+0.12))
Performance:
• The area under the ROC curve is 0.72 for 6-month survival.
