Islam et al reported the Goteborg University Cirrhosis Index (GUCI) for identifying significant hepatic fibrosis in a patient with viral hepatitis C. The authors are from Goteborg University in Sweden.
Significant fibrosis includes bridging fibrosis and cirrhosis.
Parameters:
(1) serum AST in U/L as multiple of the upper limit of normal (cutoff > 2)
(2) prothrombin-INR
(3) platelet count in 10^9/L (cutoff < 190)
GUCI =
= (serum AST as multiple of ULN) * (INR) * 100 / (platelet count)
Interpretation:
• A GUCI score > 1 is associated with significant fibrosis.
• A GUCI score > 4 had a positive predictive value of 100% for cirrhosis.
• A GUCI score < 0.2 excludes significant fibrosis.
