Description

Dente et al developed a massive blood transfusion protocol for use in the resuscitation of certain patients. They identified patients with civilian gunshot wounds to the torso who may require the use of the protocol. The authors are from Emory University and Grady Memorial Hospital in Atlanta.


 

Patient selection: torso gunshot wound

 

Massive transfusion protocol: described in Table 1, page 299

 

Indications for use of the massive transfusion protocol - one or more of the following:

(1) high risk trajectory (transpelvic, multicavitary)

(2) initial systolic blood pressure < 90 mm Hg

(3) initial base deficit < -10 mmol/L (implying acidosis)

 

If none of these indications were present then the anatomic extent of the injuries is determined prior to starting the transfusion protocol.
