The pleomorphic pattern is a rare form of lobular carcinoma which has more pleomorphic nuclei and a worse prognosis than the classic type.
Histologic features of nuclei from a pleomorphic tumor as compared to the classic lobular pattern:
(1) larger nuclei
(2) greater irregularity in the nuclear contour
(3) higher mitotic rate
(4) more prominent nucleoli
(5) more hyperchromatic
(6) more chromatin clumping
A tumor should have >= 50% pleomorphic nuclei to be termed pleomorphic (Weidner and Semple).
Clinical features of a pleomorphic lobular carcinoma:
(1) shorter recurrence free survival
(2) greater risk of recurrence
(3) more aggressive course
