Xanthgranulomatous cholecystitis (XGC) is associated with xanthogranulomas in the wall of the gallbladder. Kwon et al identified different histologic grades of xanthogranulomatous cholecystitis. The authors are from the Kansai Medical University in Osaka, Japan.
Histologic components:
(1) foamy histiocytes and xanthoma cells
(2) foreign body type giant cells
(3) acute inflammatory cells (early)
(4) fibrosis (later stages)
Parameters:
(1) degree of infiltration by foamy histiocytes and foreign body type giant cells
(2) degree of fibrosis

The presence of dense adhesions (which may be associated with Grade 3 fibrosis of the gallbladder wall) may require an open cholecystectomy.
