Monica et al reviewed indications for the early diagnosis of cerebral palsy (CP). Various indications exist, depending on age and circumstances. The authors are from the University of Sydney in Australia.
Patient selection: older infant or toddler
Triggers for suspecting cerebral palsy may be some neurologic deficit such as inability to sit at 9 months of age or hand asymmetry.
Screen for early signs of cerebral palsy:
(1) early handedness
(2) stiffness or tightness of legs
(3) persistent fisting of the hands
(4) persistent head lag
(5) inability to sit without support after 9 months of age
(6) any asymmetry in posture or movement
(7) persistent Moro reflex
(8) toe-walking or asymmetric walking beyond 12 months
Examination:
(1) review medical history
(2) Hammersmith Infant Neurological Examination
(3) MRI of the brain
(4) some form of developmental assessment
(5) motor test (Alberta Motor Scale; Motor Assessment of Infants; Neuro Sensory Motor Development Assessment)
