Number of needle punctures required to insert the catheter

Was there any deficiency with the skin preparation prior to the insertion?

Was there a modification of the initial insertion site?

Did the inserted portion of the catheter come in contact with a nonsterile surface prior to placement?

Has bleeding or other problem developed at the insertion site?

Did any other problems occur during the insertion?

Is the patient expected to receive paranteral nutrition within the next month?
