Peth reported possible causes of medication errors in the Emergency Department. These are divided along the stages of the medication process, ending with monitoring the patient. The authors are from the University of Missouri.
Errors in drug monitoring:
(1) failing to monitor the patient long enough for signs of allergy or adverse effects
(2) failing to know possible adverse effects of a medication
(3) failing to inform the patient and family about possible adverse effects
(4) failing to provide simple but sufficiently detailed written instructions about the medications
(5) failing to emphasize the need to monitor the effects of medications when appropriate
(6) failing to arrange follow-up of the patient (to include review the effectiveness or adverse effects of medications)
(7) failing to notify the patient's primary care provider of changes in drug regimen
