Gastrointestinal, hepatic and/or nutritional disorders:
(1) total parenteral nutrition (TPN)
(2) starvation
(3) celiac disease
(4) bariatric surgery
(5) short bowel syndrome
(6) pancreatectomy
(7) viral hepatitis C
(8) acute fatty liver of pregnancy
(9) HELLP syndrome
Endocrine disorders:
(1) polycystic ovary disease
(2) hypothyroidism
(3) growth hormone deficiency or other hypopituitary syndrome
(4) hyperprolactinemia
(5) Cushing's syndrome
(6) hyperaldosteronism
Hereditary disorders:
(1) disorders of lipid storage, including Niemann-Pick disease type C (3)
(2) disorders of lipoprotein metabolism (4)
(3) disorders of amino acid metabolism (3)
(4) disorders of glycosylation (5)
(5) Wilson's disease
(6) alpha-a antitrypsin deficiency
(7) NBAS deficiency
(8) inborn errors of bile acid synthesis
