Myelodysplasia cutis is a rare skin disorder that can occur in patients with myelodysplasia.
Clinical findings:
(1) typically seen in an older adult
(2) erythematous plaques
(3) diffuse papulonodules
Criteria for the diagnosis of myelodysplasia cutis:
(1) presence of myelodysplasia or myeloproliferative neoplasm
(2) atypical immature myeloid cells in the skin. some of which may be described as histiocytoid
(3) presence of the same genetic mutation(s) in skin lesions and bone marrow
Immunoperoxidase findings:
(1) myeloperoxidase positive
(2) positive for: CD33, CD163, CD14, CD68
(3) low Ki-67
(4) negative for CD34, CD117, CD56
The presence of skin lesions may precede the bone marrow manifestations by several months or years. If a bone marrow collected at the time of MDS cutis diagnosis is normal, then the patient should be followed for marrow involvement.
Differential diagnosis:
(1) histiocytoid Sweet syndrome
(2) leukemia cutis
