Description

Bradley et al outlined the management of an abscess or necrotizing pneumonia in a pediatric patient with community-acquired pneumonia (CAP). In most cases intravenous antibiotic therapy will be adequate therapy. The authors are from the Pediatric Infectious Diseases Society and the Infectious Diseases Society of America.


 

Patient selection: pediatric patient with CAP an abscess or necrotizing pneumonia

 

The first step is to perform a CT scan with contrast to characterize the process.

 

The mainstay of therapy is medical management with intravenous antibiotics selected based on susceptibility testing.

 

Special Hanlding

Management May Include

peripheral abscess without connection to bronchial tree

image-guided needle drainage

underlying pulmonary anomaly or lesion (sequestration, malformation, etc)

surgical consultation

 

Complications of overlying aggressive management:

(1) bronchopleural fistula
