Sonis et al developed the Oral Mucositis Assessment Scale (OMAS) for evaluating oral mucositis following radiation and/or chemotherapy. This can be used to monitor a patient over time and to assess the effectiveness of interventions. The authors are from multiple institutions in the United States, Canada and the Netherlands.
Parameters:
(1) ulceration and/or pseudomembrane
(2) erythema
Sites assessed:
(1) upper lip
(2) lower lip
(3) right cheek
(4) left cheek
(5) right ventral and lateral tongue
(6) left ventral and lateral tongue
(7) floor of mouth
(8) soft palate and fauces
(9) hard palate

score for each site =
= (points for ulceration/pseudomembrane) + (points for erythema)

= SUM(points for all 9 sites)
Interpretation:
• minimum site score: 0
• maximum site score: 5
• minimum total score: 0
• maximum total score: 45
• The higher the scores, the more severe the mucositis.
