Description

Ostgathe et al used a screening tool to identify the need for specialized palliative care in a patient with cancer. The authors are from University Hospital Erlangen in Germany.


Parameters:

(1) diagnosis

(2) ECOG functional status

(3) complications of advanced cancer associated with life-expectancy less than 12 months (brain metastases, cachexia, etc)

(4) severe comorbid disease with life-expectancy less than 12 months (end-stage renal disease, cirrhosis, etc)

(5) control of symptoms by standard approaches

(6) distress related to diagnosis or therapy

(7) concerns about course of disease and decision making

(8) patient/family request for palliative care consultation team

(9) need of team for assistance with complex decision making or determining goal of care

(10) length of stay

The scoring for the diagnosis could be pulled out if advanced cancer is the condition for performing the screen.

A life expectancy less than 12 months replaces asking "surprise" questions.

Interpretation:

minimum score: 0

maximum score: 14

A score >= 5 was associated with a need for specialized palliative care.
