A patient with granulomatosis with polyangiitis may develop penile ulcerations.
The diagnosis is straightforward in a patient with known granulomatosis with polyangiitis but can be more challenging when penile lesions are the initial presentation.
Diagnosis:
(1) vasculitis in ulcer biopsy
(2) positive for c-ANCA
(3) exclusion of other diagnoses
Other findings may include:
(1) palpable purpura
(2) urethral stricture
(3) lesions in the usual sites of occurrence: lungs, sinuses, and/or kidneys
