The performance rate and exception rate for an intervention can be reported for a physician based on reported data.
Parameters:
(1) number of patients who received the recommended intervention
(2) number of patients who did not get the intervention because of a valid medical reason (medical exception)
(3) number of patients who did not get the intervention because of a patient-related factor (patient-related exception)
(4) number of patients who did not get the intervention for other reasons
total number of patients with condition =
= SUM(number of patients in each of the 4 groups)
number of patients eligible for the intervention =
= (total number of patients) - (patients with a medical exception) - (patients with a patient-related exception) =
= (number of patients who received the intervention) + (number of patients who did not get the intervention and who did not have an exception)
performance rate =
= (number of patients given the intervention) / (number of patients eligible for the intervention)
medical-exception rate =
= (number of patients with a medical exception) / (total number of patients)
patient-related exception rate =
= (number of patients with a patient-related exception) / (total number of patients)
percent of patients who were eligible =
= (number of patients eligible for the intervention) / (total number of patients)
where:
• A valid medical reason would include contraindication or potential drug interaction.
• A patient-related factor may be related to preference, failure to respond or other reason.
• In theory a patient could have both a medical exception and a patient-related exception.
• The denominators for performance rate and for the exception rates are different.
