Bennett et al reported hyperlactatemia and other predictors for in-hospital mortality for a patient with Type A aortic dissection undergoing surgery. The authors are from Vanderbilt University.
Patient selection: acute type A aortic dissection
Risk factors for in-hospital mortality:
(1) elevated serum lactic acid concentration, >= 6.0 mmol/L (OR 1.5)
(2) effusion (OR 4.8)
(3) neurologic change (OR 5.3)
(4) severe aortic regurgitation (6.8)
The severity of the malperfusion syndrome (cerebral, cardiac, visceral) also impact outcomes.
