Lekovic et al reported a predictive score for distinguishing prefibrotic primary myelofibrosis (prePMF) from essential thrombocythemia. These 2 myeloproliferative disorders may have a similar presentation with thrombocytosis. The authors are from the University of Belgrade and Wroclaw Medical University.
Patient selection: either pre-fibrotic myelofibrosis or essential thrombocythemia
Parameters:
(1) age of the patient in years
(2) splenomegaly
(3) serum LDH concentration

Interpretation:
• minimum score: 0
• maximum score: 4
• A score >= 2 has a sensitivity of 0.70 and specificity of 0.82, with the highest Youden index. The positive predictive value of a score >= 2 is 0.70 in the training and 0.62 in the validation cohorts.
