Description

Rombout-Sestrienkova et al reported predictive models for estimating the therapy required for the initial management of a patient with untreated hemochromatosis. One model predicts the number of phlebotomy units required. The authors are from multiple institutions in The Netherlands.

Patient selection: untreated hemochromatosis

Management: each phlebotomy treatment procedures should remove 1 unit of whole blood (450 to 500 mL).

Goal: serum ferritin <= 100 µg/L

Parameters:

(1) initial serum ferritin in µg/L

(2) initial hemoglobin in mmol/L

(3) body mass index (BMI) in kg per square meter

(4) age in years

Parameter

Points

serum ferritin

0.011 * (ferritin)

initial hemoglobin

-0.7 * (hemoglobin)

BMI

-0.55 * (BMI)

age

0.16 * (age)

where:

• 1 mmol/L hemoglobin equals 1.61 g/dL.

number of phlebotomy treatment procedures =

= SUM(points for all of the parameters) + 19.8
