Hohlfelder et al reported factors associated with a larger change in INR after stopping an infusion of bivalirudin. The authors are from Brigham and Women's Hospital in Boston.
Patient selection: infusion of bivalirudin
The change in INR after stopping bivalirudin is the INR during therapy minus the INR after stopping the infusion.
Predictors from multivariate analysis for a greater drop in INR:
(1) higher serum creatinine (suggesting drug accumulation during therapy)
(2) greater change in INR after starting bivalirudin
(3) high final bivalirudin rate in mg per kg per hour
