Description

Carena et al identified factors associated with multidrug-resistant Gram-negative bacteremia in a cancer patient. A patient with a resistant organism may require careful selection of antibiotic therapy. The authors are from multiple institutions in Argentina and members of the Argentine Group for the Study of Bacteraemias in Cancer and Stem Cell Transplant (ROCAS).


Patient selection: adult with cancer and bacteremia due to a Gram-negative bacteria

 

Predictor of infection with a multidrug-resistant organism:

(1) recent antibiotic use (within past 30 days AND for > 48 hours; OR 2.8)

(2) recent ICU admission (within past 14 days AND for >= 72 hours; OR 2.9)

(3) hospitalization for >= 7 days prior to onset of bacteremia (OR 3.5)

(4) severe mucositis (OR 5.3)

(5) recent/previous colonization/infection with a multidrug-resistant Gram-negative bacteria (OR 2.3)

 

The risk increases with the number of risk factors present.
