Cohen et al identified risk factors predictive of Candida glabrata fungemia in nonneutropenic patients in the intensive care unit (ICU). These factors can help to identify a patient who should be considered for this diagnosis. The authors are from multiple hospitals in France.
Patient selection: ICU patients without neutropenia (neutropenia defined as an absolute neutrophil count < 500 per µL) and not colonized with a Candida species
Risk factors for Candida glabrata fungemia:
(1) age > 60 years
(2) recent abdominal surgery
(3) solid tumor
(4) recent therapy with third-generation cephalosporin (during current hospitalization)
(5) time interval from ICU admission to first positive blood culture <= 7 days
(6) negative for diabetes mellitus
where:
• The odds ratios for the first 5 risk factors range from 4.5 to 6.9.
• The odds ratio for diabetes was 0.11. The odds ratio for no diabetes is 9.1.
