Harteman et al identified findings in a mother or neonate which may be indicators of arterial ischemic stroke in a neonate. The authors are from Wilhelmina Children's Hospital, University Medical Center, Utrecht, The Netherlands.
Patient selection: full-term neonate
Target: perinatal arterial ischemic stroke (PAIS)
Predictors from multivariate analysis:
(1) maternal fever > 38°C (odds ratio 10.2)
(2) early-onset sepsis (EOS) and/or meningitis (odds ratio 5.8)
(3) hypoglycemia < 2 mmol/L (odds ratio 13.0)
(4) Apgar score at 5 minutes < 7 (odds ratio 18.1)
Additional findings seen in the univariate analysis:
(1) fetal heart rate decelerations
(2) meconium-stained amniotic fluid
(3) emergency caesarean section
(4) umbilical artery pH < 7.10
(5) Apgar score at 1 minute <= 3
